Referral
Access Personal Support accepts referrals from a wide range of sources, including healthcare professionals such as case managers and occupational therapists, as well as legal representatives, insurers, institutions, and facilities. We also partner with programs such as Veterans Affairs.
Services are provided across Ontario to seniors, individuals recovering from motor vehicle accidents (MVAs), those living with acquired brain or spinal injuries, those experiencing mental health challenges, or managing complex or chronic health needs.
Referral Process
Any relevant documentation or medical history should be emailed to info@apsont.com.
A list of suitable workers and their curriculum vitae will be provided to the referral source. Attention is focused on ensuring that staff approach, skills and location best suit the needs of the specified individual. Once a suitable match is established, a personalized care plan is developed to address the individual’s needs.
A meeting is arranged between the person receiving care and the support worker, and may also include family members or other healthcare professionals.
After services are initiated, documentation is completed at the end of each session, and ongoing communication is maintained with the treatment team and family members involved.
